A Care Plan is document control, not editing. Nothing is changed in place — every update leaves a trail a reviewer can follow.
The document is reissued at the next revision number rather than edited in place, so the version your client approved still exists and the change is visible.
A new line on the document itself: the date, what changed, the reason it changed — a legislation update, a client standard, a change on your site — and who approved it.
Page one carries the current revision, issue date and next review date. The master register is updated at the same time so the whole suite stays consistent.
After every scheduled review you get a summary listing each document touched, what changed in it and why — issued to your nominated document owner.

A document you don’t have yet is quoted as new work — writing one isn’t maintenance — and it carries the same 90-day Review-Pass Guarantee as the original pack. Once issued it joins your numbering scheme and your register. Your included ad-hoc updates cover changes to documents you already have.
Nothing is deleted. A document that is no longer needed is marked superseded — naming the document that replaces it — or withdrawn, with the date and reason in its revision history. The register keeps the entry with its status changed, and we tell your document owner which controlled copies to pull from circulation.
Your Care Plan fee is set from the active document count at each renewal, so it moves with your suite in both directions. Retiring a document is included at every tier.

A Document Care Plan keeps the suite current after delivery, at three levels: Bronze — one scheduled review a year; Silver — two scheduled reviews a year; Gold — a scheduled review every quarter, plus monitoring of the legislation and client standards your documents cite.
